Calling All Dads! — HFA Needs Your Opinion

Dads in Action
March 29, 2009

The Hemophilia Federation of America (HFA) offers the Dads in Action (DIA) program as an educational and social support system aimed at encouraging and teaching fathers of children with hemophilia and fathers with hemophilia to build stronger bonds with their children and families.

In an effort to identify ways to best serve and support the Dads in our community, we ask for your input. Please take a few minutes to complete this survey about the Dads in Action program offered by the Hemophilia Federation of America (HFA). We are always striving for ways to better meet your needs and those of your family so your input is greatly appreciated.

The survey collects information in four key areas:

  • the information and services Dads need;
  • how best to deliver those services;
  • living with and caring for bleeding disorders, and;
  • staying involved with DIA.

Please use this link to access the survey: https://www.resultsonline2.net/survey/index.php?id=1595af6435015c77a7149e92a551338e

The survey will take approximately 15 minutes to complete. Please answer the questions honestly and completely.

All questions must be answered to successfully complete the survey.

Your responses will remain anonymous.

The survey will close Monday, April 6, 2009.

Please direct any questions to Michelle Burg, HFA Program Manager at: m.burg@hemophiliafed.org or 262.490.2221.

Thanks again for helping us help you!
